| In order to provide services to our clients, we will need to have the following information from all vendors: |

| Insurance: Insurance certificate sent from your insurance agent outlining the coverage you have. Workers Compensation insurance coverage needs to be provided and the coverage must be at least the minimum required by statute. |

| References: For new vendors, we will need to have 3 references from work you have performed. |
| |
| Licenses: Appropriate business license from appropriate regulatory agency. |
